Overweight and obesity are defined as abnormal or excessive fat accumulation that presents a risk to health. A crude population measure of obesity is the body mass index (BMI), a person’s weight (in kilograms) divided by the square of his or her height (in metres). A person with a BMI of 30 or more is generally considered obese. A person with a BMI equal to or more than 25 is considered overweight.
Overweight and obesity are major risk factors for a number of chronic diseases, including diabetes, cardiovascular diseases and cancer. Once considered a problem only in high income countries, overweight and obesity are now dramatically on the rise in low- and middle-income countries, particularly in urban settings.

What are overweight and obesity?

Overweight and obesity are defined as abnormal or excessive fat accumulation that may impair health.
Body mass index (BMI) is a simple index of weight-for-height that is commonly used to classify overweight and obesity in adults. It is defined as a person's weight in kilograms divided by the square of his height in meters (kg/m2).
The WHO definition is:
  • a BMI greater than or equal to 25 is overweight
  • a BMI greater than or equal to 30 is obesity.
BMI provides the most useful population-level measure of overweight and obesity as it is the same for both sexes and for all ages of adults. However, it should be considered a rough guide because it may not correspond to the same degree of fatness in different individuals.

Facts about overweight and obesity

Overweight and obesity are the fifth leading risk for global deaths. At least 2.8 million adults die each year as a result of being overweight or obese. In addition, 44% of the diabetes burden, 23% of the ischaemic heart disease burden and between 7% and 41% of certain cancer burdens are attributable to overweight and obesity.
Some WHO global estimates from 2008 follow.
  • 1.5 billion adults, 20 and older, were overweight.
  • Of these 1.5 billion overweight adults, over 200 million men and nearly 300 million women were obese.
  • Overall, more than one in ten of the world’s adult population was obese.
In 2010, around 43 million children under five were overweight. Once considered a high-income country problem, overweight and obesity are now on the rise in low- and middle-income countries, particularly in urban settings. Close to 35 million overweight children are living in developing countries and 8 million in developed countries.
Overweight and obesity are linked to more deaths worldwide than underweight. For example, 65% of the world's population live in countries where overweight and obesity kill more people than underweight (this includes all high-income and most middle-income countries).

What causes obesity and overweight?

The fundamental cause of obesity and overweight is an energy imbalance between calories consumed and calories expended. Globally, there has been:
  • an increased intake of energy-dense foods that are high in fat, salt and sugars but low in vitamins, minerals and other micronutrients; and
  • a decrease in physical activity due to the increasingly sedentary nature of many forms of work, changing modes of transportation, and increasing urbanization.
Changes in dietary and physical activity patterns are often the result of environmental and societal changes associated with development and lack of supportive policies in sectors such as health, agriculture, transport, urban planning, environment, food processing, distribution, marketing and education.

What are common health consequences of overweight and obesity?

Raised BMI is a major risk factor for noncommunicable diseases such as:
  • cardiovascular diseases (mainly heart disease and stroke), which were the leading cause of death in 2008;
  • diabetes;
  • musculoskeletal disorders (especially osteoarthritis - a highly disabling degenerative disease of the joints);
  • some cancers (endometrial, breast, and colon).
The risk for these noncommunicable diseases increases, with the increase in BMI.
Childhood obesity is associated with a higher chance of obesity, premature death and disability in adulthood. But in addition to increased future risks, obese children experience breathing difficulties, increased risk of fractures, hypertension, early markers of cardiovascular disease, insulin resistance and psychological effects.

Facing a double burden of disease

Many low- and middle-income countries are now facing a "double burden" of disease.
  • While they continue to deal with the problems of infectious disease and under-nutrition, they are experiencing a rapid upsurge in noncommunicable disease risk factors such as obesity and overweight, particularly in urban settings.
  • It is not uncommon to find under-nutrition and obesity existing side-by-side within the same country, the same community and the same household.
Children in low- and middle-income countries are more vulnerable to inadequate pre-natal, infant and young child nutrition At the same time, they are exposed to high-fat, high-sugar, high-salt, energy-dense, micronutrient-poor foods, which tend to be lower in cost. These dietary patterns in conjunction with low levels of physical activity, result in sharp increases in childhood obesity while undernutrition issues remain unsolved.

How can overweight and obesity be reduced?

Overweight and obesity, as well as their related noncommunicable diseases, are largely preventable. Supportive environments and communities are fundamental in shaping people’s choices, making the healthier choice of foods and regular physical activity the easiest choice, and therefore preventing obesity.
At the individual level, people can:
  • limit energy intake from total fats;
  • increase consumption of fruit and vegetables, as well as legumes, whole grains and nuts;
  • limit the intake of sugars;
  • engage in regular physical activity;
  • achieve energy balance and a healthy weight.
Individual responsibility can only have its full effect where people have access to a healthy lifestyle. Therefore, at the societal level it is important to:
  • support individuals in following the recommendations above, through sustained political commitment and the collaboration of many public and private stakeholders;
  • make regular physical activity and healthier dietary patterns affordable and easily accessible too all - especially the poorest individuals.
The food industry can play a significant role in promoting healthy diets by:
  • reducing the fat, sugar and salt content of processed foods;
  • ensuring that healthy and nutritious choices are available and affordable to all consumers;
  • practicing responsible marketing;
  • ensuring the availability of healthy food choices and supporting regular physical activity practice in the workplace.

What are the health consequences of being overweight?

Q: What are the health consequences of being overweight?

A: The latest WHO projections indicate that at least one in three of the world's adult population is overweight and almost one in 10 is obese. Additionally there are over 20 million children under age five who are overweight.
Being overweight or obese can have a serious impact on health. Carrying extra fat leads to serious health consequences such as cardiovascular disease (mainly heart disease and stroke), type 2 diabetes, musculoskeletal disorders like osteoarthritis, and some cancers (endometrial, breast and colon). These conditions cause premature death and substantial disability.
What is not widely known is that the risk of health problems starts when someone is only very slightly overweight, and that the likelihood of problems increases as someone becomes more and more overweight. Many of these conditions cause long-term suffering for individuals and families. In addition, the costs for the health care system can be extremely high.
The good news is that overweight and obesity are largely preventable. The key to success is to achieve an energy balance between calories consumed on one hand, and calories used on the other hand.
To reach this goal, people can limit energy intake from total fats and shift fat consumption away from saturated fats to unsaturated fats; increase consumption of fruit and vegetables, as well as legumes, whole grains and nuts; and limit their intake of sugars. And to increase calories used, people can boost their levels of physical activity - to at least 30 minutes of regular, moderate-intensity activity on most days.

What is Visceral Fat?

Visceral fat [vis-er-uhl] is the official name for the fat inside a big “beer belly”.  It is not the same as subcutaneous fat [suhb-kyoo-tey-nee-uhs] found under the skin. Visceral fat and subcutaneous fat may, or not, appear together. Subcutaneous fat is stored in the hips, thighs, butt, back, and back of the arms.  Most of the body's fat is subcutaneous, especially in women. 
Visceral fat is a real trouble maker. It causes metabolic syndrome and chronic inflammation that leads to heart disease and diabetes.  Visceral fat literally encrusts the vital organs, the kidneys, liver, stomach, and others.  The abdominal cavity that houses the organs is called the “viscera” and those are the “visceral organs" -  buried in “visceral fat".
Visceral fat contributes to high blood pressure by squeezing the kidneys, working them and wearing them out.  It also drains directly into the liver where it infiltrates, replacing functional tissue with fat.  Research in a large urban center in the United States found "fatty liver" in one-third of adults surveyed.  Fatty liver can lead to cirrhosis.

From Whence It Came

Visceral fat is related to eating too many easily digested carbohydrates - simple sugars and processed starches - and then by not burning off the glucose they supply.  Visceral fat is biochemically programmed to take up extra glucose and turn it into even more visceral fat!  Its job is to take carbohydrates out of the system when the liver stores are full.  Studies also suggest that saturated fat increases visceral fat but unsaturated fat does not.  When it comes to beer, a study of alcohol showed that a pattern of drinking large amounts of any alcohol at once (binge drinking) leads to visceral fat but daily drinking of small amounts does not.

A Measure of Health


Visceral fat is a predictor of health risk. Measure visceral fat (with a tape measure) to see if you have too much.  It cannot be measured by the bathroom scale, by the special scale that measures body-fat, nor the BMI chart.  The best way to measure visceral fat is by CT scan or MRI, but they are too expensive for routine use.
Waist circumference is the proxy measure for visceral fat. To take the measurement, use a flexible tape, stand up straight, and breathe naturally.  Place the flat tape around the bare abdomen in line with the navel.  Do not compress the skin.

A waist circumference of 35 inches (88 cm) or more for a woman, and 40 inches (102 cm) or more for a man, is a sign of too much visceral fat. 

Note: Waist circumference is an important measure of visceral fat in men, although in women, a larger waist has more to do with subcutaneous fat.  A BMI over 35 negates the usefulness of waist circumference for both sexes.
Melt Visceral Fat
To shed visceral fat, stop the cycle of eating too many calories, carbohydrates and fats, and then burn the visceral fat for energy to fuel physical activity.  Cardio-type exercise has been shown to melt away visceral fat.  Even modest exercise, like walking 30-45 minutes five days a week, can make a big difference.
Research shows the Mediterranean Diet eaten within a calorie budget does a good job of reducing visceral fat.  Mediterranean food is primarily plants – vegetables and fruit, beans, whole grains, and olives and nuts - along with fish and a bit of red meat.

The Importance of Body Fat Percentage

Did you know… That two people can have the same height and weight, but very different percentages of body fat?

Did you know… That as we age, we tend to gain fat around our organs (“visceral fat”) that can’t be detected by measuring “skin folds” or even with a scale?

Did you know… That chances are that if you are losing weight, you will lose muscle as well as fat?

Weight management is a goal on a lot of people’s minds these days. People get on the scales in the morning, and the number they see may determine how they feel about themselves. But the truth is that some people make think they are overweight, when the amount of fat they have on their bodies is just fine, while others may carry hidden fat that the scale won’t tell them about.

What is Body Fat Percentage?

Our bodies are made up of a lot of different kinds of tissues (plus a lot of water). There is muscle, fat, bone, and specialized tissue such as is in our various organs. The body fat percentage is just that –- the percentage of our weight which is made up of fat. Body fat percentage is similar to terms such as body fat ratio and body composition. The part that isn’t fat is called our “lean body mass.” A 160-pound person with a lean body mass of 120 lbs has 25 percent body fat (40 lbs of fat). That may sound like a lot, but our bodies need a certain amount of fat for insulation, energy storage, hormone production, and other functions scientists are just discovering.

How Much Body Fat Should a Person Have?

This depends upon a few variables. Women need more body fat than men. Breasts are almost all fat, and women have more fat around their hips -– that’s just the way it was meant to be. If a woman’s body fat drops too low, she will stop menstruating. Also, men naturally have a greater amount of muscle bulk. Another factor is age. As people get older, their muscles tend to shrink, and they tend to accumulate visceral fat. This is at least partially related to the decline in testosterone and other hormones. You can find your individual body fat recommendation in these charts. It is important not to confuse BMI with body fat. The BMI (body mass index) numbers are NOT percentages of body fat. I often see advice on Web sites where the two are plainly being confused. For example, a BMI of 29 is in the overweight range for women. This has nothing to do with 29 percent body fat, which is considered healthy.

What’s the Difference between Body Fat Percentage and BMI?

BMI (body mass index) is a formula based on height and weight. It was developed because in the general population, it is correlated with body fat. However, there are quite a few groups of people for whom BMI is not as accurate –- short women and muscular people, to name two. BMI also varies according to some ethnic groups. Also, for people who are interested in changing their body composition and not just their weight, knowing body fat percentage is an improvement over BMI. For example, if you are exercising to build muscle (a good goal), knowing your body fat percentage is a good idea. Also, when losing weight, you want to preserve as much lean body mass as possible. (Low-carb diets generally produce better results than high carb ones for this purpose.)

As far as I can tell, this is the most authoritative, as it is based on World Health Organization and National Institutes of Health recommendations, and takes age into account. Please do not confuse these numbers with the BMI (Body Mass Index) scores, which are not the same thing.
